Community Health Facilitators — Field Stories

Everyday outreach by Community Health Facilitators — adolescent health and equality sessions, routine immunization support, antenatal home visits, infant-nutrition counselling, NCD awareness for the elderly and menstrual-hygiene sessions in schools.

Through regular village-level outreach, the CHF engaged adolescents in an open discussion on health, nutrition and equality. The session encourages young people to understand the importance of healthy behaviour and equal opportunities for boys and girls in the community. Working closely with government health workers, the CHF supports the routine immunization programme. These outreach activities help improve awareness among families and ensure better access to essential healthcare services. CHF conducting an interactive session with school children on nutrition, personal health and gender equality. Through regular school visits, adolescents are encouraged to adopt healthy practices and build positive social values from an early age. The Community Health Facilitator visiting a pregnant woman at her home to provide guidance on healthy nutrition, antenatal care, immunization and essential supplements during pregnancy. Regular counselling sessions help ensure better care and wellbeing for both mother and child. Supporting government frontline health workers during an immunization session in the community. The CHF helps families get timely vaccination services and stronger healthcare support at the village level. CHF reaches families at the grassroots level through regular health visits. This interaction focused on infant nutrition, breastfeeding practices and family planning. Through every visit, the aim is to build confidence, awareness and healthier futures. CHF engaged with older people at her Health Resource Centre (HRC) to discuss NCDs and preventive healthcare. It highlights the importance of regular BP and diabetes check-ups, as prevention is better than cure. Team members from the head office visit the CHF field area to understand field-level work and challenges. They also interact with beneficiaries to assess their awareness about health and healthcare services. Such visits help improve support systems and strengthen community engagement. A Community Health Facilitator visiting schools to talk openly about menstrual hygiene and reproductive health with young schoolgirls. These interactive sessions create a safe space for adolescent girls to break taboos and ask important questions. With proper knowledge about hygiene & rights, these young girls can focus on their studies & future.
